About Dr. Ramnarayan

Dr. Ramnarayan is a Consultant Neurosurgeon at Muscat Private Hospital with specialised expertise in functional and restorative neurosurgery — the branch of neurosurgery concerned not with removing disease, but with restoring function that has been lost or disrupted by neurological illness or injury.

Rather than conventional open surgery, much of his practice involves neuromodulation: the implantation of precisely targeted devices that deliver controlled electrical stimulation to the brain, spinal cord or peripheral nerves, or pumps that deliver medication directly to the fluid around the spinal cord. These techniques are typically considered when medication alone has stopped controlling a condition adequately, and they are adjustable and, in most cases, reversible.

His procedures include deep brain stimulation (DBS) for movement disorders such as Parkinson’s disease, dystonia and essential tremor; spinal cord stimulation (SCS) for chronic neuropathic pain and failed back surgery syndrome; vagus nerve stimulation (VNS) for drug-resistant epilepsy; intrathecal baclofen and morphine pump therapy for severe spasticity and intractable pain; and sacral nerve stimulation for bladder and bowel dysfunction.

Dr. Ramnarayan holds an MCh in neurosurgery, FRCS, and is an International Fellow of the American Association of Neurological Surgeons (IFAANS). He completed a Functional Neurosurgery Fellowship in the United Kingdom and has managed complex neurosurgical and neurofunctional cases across leading healthcare institutions. He consults in English, Hindi, Malayalam and Tamil.

Care offered

Procedures & Services

  • Deep Brain Stimulation (DBS)

  • Implantation of fine electrodes into precisely targeted areas of the brain, connected to a pulse generator placed under the skin. Used principally for Parkinson’s disease, dystonia, essential tremor and drug-resistant epilepsy, where symptoms are no longer adequately controlled by medication.

  • Spinal Cord Stimulation (SCS)

  • Implantation of electrodes in the epidural space to modify pain signals travelling to the brain. Used for failed back surgery syndrome, severe diabetic neuropathy, brachial plexus injury pain, refractory angina, and peripheral vascular disease where the aim is limb salvage.

  • Vagus Nerve Stimulation (VNS)

  • Implantation of a device that delivers regular stimulation to the vagus nerve in the neck, used in drug-resistant epilepsy to reduce seizure frequency and severity.

  • Intrathecal Pump Therapy

  • An implanted pump delivers medication directly into the fluid surrounding the spinal cord, allowing far smaller doses and fewer side effects than tablets or injections. Baclofen pumps are used for severe spasticity; morphine pumps for cancer pain, failed back surgery syndrome and other intractable chronic pain.

  • Sacral Nerve Stimulation (SNS)

  • Stimulation of the sacral nerves to restore control of bladder and bowel function, used for urinary and faecal incontinence, non-obstructive urinary retention, and selected cases of constipation, irritable bowel syndrome and chronic pelvic pain.

  • Emerging and Highly Selected Indications

  • Neuromodulation is also being applied to a small number of conditions beyond its established uses. Dr. Ramnarayan has experience in these areas, which include deep brain stimulation for severe treatment-resistant psychiatric illness

  • These are specialist applications considered only in carefully selected patients, after all conventional treatments have been exhausted and following full multidisciplinary assessment. Suitability, the strength of the available evidence and the likely benefits and risks are discussed individually at consultation. They are not offered as routine treatments.

Common Questions

Frequently Asked Questions

Functional neurosurgery aims to restore function rather than remove disease. Instead of operating to take something out, it uses precisely targeted implanted devices to modify the activity of the nervous system — for example to reduce tremor, control seizures, relieve severe spasticity or block chronic pain.
DBS involves implanting fine electrodes into specific areas of the brain, connected to a small pulse generator under the skin. It is used mainly for Parkinson’s disease, dystonia, essential tremor and drug-resistant epilepsy, when medication is no longer controlling symptoms well enough. The stimulation can be adjusted afterwards and, in most cases, switched off or removed.
Often, yes. Spinal cord stimulation is used for failed back surgery syndrome, severe diabetic neuropathy and nerve injury pain, and intrathecal morphine pumps deliver pain relief directly to the spinal fluid for cancer pain and other intractable pain — usually with far smaller doses and fewer side effects than tablets.
An intrathecal baclofen pump can substantially reduce severe spasticity from cerebral palsy and other causes of brain or spinal injury. Whether it is appropriate depends on a full assessment, and a trial dose is normally given before a pump is implanted.
